COMMITMENT TO CONFIDENTIALITY, PROFESSIONAL SECRECY, DISCRETION AND PROTECTION OF PERSONAL DATA
As part of the service provision agreement that binds the parties, this amendment aims to ensure the obligations of confidentiality and protection of personal data processed by HAKIM DOCTOR

1. Privacy clause
​
The service provider is bound by a strict duty of confidentiality. He hereby irrevocably undertakes with regard to patients, not to disclose any confidential information concerning them and to observe strict discretion as to the information he may collect. In addition, he undertakes not to disclose, under any circumstances, the personal data processed.
The information collected will therefore be used exclusively with discretion and confidentiality, only the necessary information will be obtained and transmitted as part of the medical consultation.
This obligation of confidentiality also includes, but is not limited to, information relating to patients, including personal data of patients.

2. The service provider's commitments with regard to the processing of personal data.

The service provider undertakes to comply with data protection legislation.
He agrees to very strict compliance with the internal procedures put in place by HAKIM DOCTOR for the protection of personal data and to report any anomaly or malfunction so that it can be corrected immediately. If necessary, he also undertakes to ensure that these procedures are respected by the members of his team and to suggest any avenues for improvement that he identifies.
It undertakes in particular and in particular to :

- Not to use the data to which he cannot access for purposes other than those provided for by my attributions.
- Only disclose this data to persons duly authorized, by reason of their functions, to receive communication, whether they are private, public, natural or legal persons.
- Do not make any copy of this data except as necessary for the performance of its functions.
- Take all measures in accordance with the practices and the state of the art within the framework of its attributions in order to avoid the diverted or fraudulent use of this data and to preserve the physical and logical security of this data.
- Ensure, within the limits of its powers, that only secure means of communication will be used to transfer this data.
- In the event of termination of his duties, return in full the data, computer files and any information medium relating to this data.

3. Duration
​
This obligation of confidentiality, discretion and protection of personal data will be observed for an indefinite period during the period of contact with patients and will remain in effect for 5 years after this period.
